Application for Membership

The undersigned applicant chapter hereby applies for membership in the Bridging Leadership Institute and requests permission to organize a Bridging Leadership Co-creation Chapter in:

City: ___________________
Municipality/City: ___________________
Date (MM/DD/YYYY): ___________________

(If this is an Online Chapter, please write 'Online' in the field next to City).

It is understood that such permission, when granted, will give this group the right to use the name, procedures, and materials of the Bridging Leadership Institute as a provisional chapter for twelve (12) months from the date received by the Bridging Leadership Institute Headquarters.

It is agreed that the right to use the Bridging Leadership Co-creation Chapter emblem, the name Bridging Leadership Co-creation Chapter, or Bridging Leadership shall be conditioned upon permission for such use being granted by the Bridging Leadership Institute.

Products downloadable from our website, www.bridgingleadershipinstitute.com, are authorized products for use in an effort to promote the Bridging Leadership Co-creation Chapter. Such use shall be discontinued if the Charter is not granted, or if the applicant chapter shall be required to do so at any time in the future, for cause, as deemed by the Bridging Leadership Institute.

Bridging Leadership Co-creation Chapters agree they must not use the trademarked and copyrighted materials of other entities in any way without express written permission from the owner. This includes, but is not limited to, use in the chapter’s name, chapter’s domain name, or other materials or publications. Once a Charter is granted, the Bridging Leadership Co-creation Chapter emblem, the name Bridging Leadership Co-creation Chapter or Bridging Leadership may be used with permission from the Trademarks team.

A minimum number of 7 individuals are required to charter a Bridging Leadership Co-creation Chapter, 4 of whom cannot belong to another Bridging Leadership Co-creation Chapter.*

Chapter officers must take necessary measures to ensure their chapter and members are in compliance with policies set by the relevant authorities.

All Bridging Leadership Co-creation Chapters must meet the following minimum requirements: meet at least twelve (12) times per year; have members participate in learning activities and practice sessions; and provide members the opportunity to develop and practice leadership skills.

Membership in a Bridging Leadership Co-creation Chapter is open to all who are interested in developing their leadership skills and/or working on community-development initiatives. No person shall be excluded from membership in a Bridging Leadership Institute Chapter, and no member shall be deliberately discriminated against in the conduct of official Bridging Leadership Institute programs because of age (except those persons under 12 years of age), race, color, creed, gender, national or ethnic origin, sexual orientation, or physical or mental disability, so long as the individual, through his or her own efforts, is able to participate in the program.

If granted, the charter and membership may be revoked by the Bridging Leadership Institute for cause, including but not restricted to: conduct unbecoming a Bridging Leadership Co-creation Chapter; failure to remain in good standing with the Bridging Leadership Co-creation Chapter; or abandonment of the Charter and membership by the applicant chapter.

As Chapter Coordinator and on behalf of the applicant chapter, I agree to the terms and conditions listed above.
